INSURANCE INFORMATION

We are in-network with the following insurances:

Blue Shield of California, United Healthcare, Oscar Insurance, and Health Net. 

*We currently have a waitlist of up to 2 months for United Healthcare. 

Coverage and benefits may vary based on your diagnosis and specific plan. Even though we are contracted with your insurance, not all plans cover nutrition visits or will only cover specific diagnosis.  We ask that you contact your insurance company to check your benefits. There are often costs associated with using insurance (co-pays, coinsurance, limited visits) and you, the client, will be responsible for those fees. 

Out-of-Network:

We are out-of-Network with many insurance providers including Anthem, Cigna, and Aetna. 

We are happy to provide you with a Superbill (session receipt) that you can submit to your insurance for potential reimbursement of your costs. It is your responsibility to inquire with your insurance company regarding benefits. Sessions will be paid at the time of service and the reimbursement from your insurance will be paid directly to you. 

BEFORE YOUR VISIT, PLEASE CALL YOUR INSURANCE COMPANY AND ASK:

We recommend calling the member services number on the back of your insurance card to ask:

  • Does my plan cover nutrition counseling sessions (specifically CPT billing codes 97802 and 97803).

  • Is Kristie Tovar Nutrition & Wellness (NPI 1255949616) covered under my plan? If not, what are my out-of network nutrition benefits?

  • Does my plan only cover medical nutrition therapy visits that are considered “medically necessary”?

  • Do I have preventative benefits for nutrition counseling?

  • Does my plan cover diagnosis code Z71.3 with CPT code 97802 or 97803?

  • Do I have a copay for in-person sessions or for telehealth sessions.

  • Do I have a deductible to meet before insurance pays? How much is my deductible?

  • How many visits are covered?

  • Do I need a physician referral or prior authorization.

Write down the date and reference number for your call and keep this number saved somewhere safe.

We will also provide a written document with this information so you can have it handy to call insurance to verify coverage.

**Our practice has a 24-hour cancellation policy. Any cancellations within less than 24 hours of your session time will result in a charge for the missed session of $175.00 -$195.00 depending on your dietitian (a full session fee) to the credit card on file.
